1. Provider-Led, Responsive Care
  • Nurse Practitioner who participates in after-hours triage.
  • Families and facilities have access to someone who can assess symptoms, make clinical decisions, and provide orders when needed.
  • We prioritize rapid response and timely signed orders, rather than leaving facilities waiting for the next business day.
  • This helps prevent unnecessary ER visits and hospitalizations while keeping patients comfortable in their preferred setting.
2. We Don’t Automatically Take Patients Off Their Medications
  • We take a patient-centered approach to medication management.
  • We do not simply come in and discontinue a patient’s medications because they are on hospice.
  • Medications are reviewed based on the patient’s goals, prognosis, comfort, and clinical needs.
  • We cover most medications related to the patient’s hospice diagnosis and plan of care, helping reduce financial and logistical burdens for families.
3. Manageable Nursing Caseloads = Better Patient Care
  • We believe hospice nursing should be about quality, not quantity.
  • Our nurses maintain manageable caseloads, allowing them to spend meaningful time with patients and families.
  • Nurses have the capacity to assess changes in condition, communicate with caregivers, coordinate with providers, and address symptoms proactively.
  • We don’t want our nurses racing from patient to patient just to meet a productivity number.
4. Our Goal Is Comfort—not Just Managing a Caseload
  • We focus on the whole patient and family, not simply the diagnosis.
  • We emphasize proactive symptom management, education, communication, and support.
  • Our interdisciplinary approach is designed to help patients remain comfortable and avoid unnecessary crises.
5. Palliative Care Doesn’t Mean Goodbye
One of our biggest differentiators is what happens when a patient no longer meets hospice criteria.
  • We offer a Palliative Care Program for ongoing symptom management.
  • Patients who “graduate” from hospice don’t simply lose their support overnight.
  • When appropriate, we can continue durable medical equipment (DME) to help provide a smoother transition.
  • Patients may continue to receive CNA services when appropriate.
  • RN and Nurse Practitioner visits are available as needed for ongoing assessment and symptom management.
  • Our philosophy is: graduating from hospice shouldn’t mean losing the relationship and support you’ve built with our team.
6. Continuity of Care
  • We want patients and families to know who they are calling and who is involved in their care.
  • Our team works to maintain continuity between the patient, family, facility, nurses, CNA, NP, and other providers.
  • We communicate changes promptly and work collaboratively with the patient’s existing healthcare team.

Our owner is a Nurse Practitioner who participates in after-hours triage and can provide clinical direction and signed orders when they are needed. We don’t believe hospice should mean automatically taking patients off medications—they deserve individualized medication management based on their goals and comfort. We intentionally keep our nurses’ caseloads manageable so they have the time to provide quality care rather than focusing on volume. And perhaps most importantly, when a patient graduates from hospice, we don’t want them to feel like they’ve been dropped. Our palliative program allows us to continue supporting appropriate patients with symptom management, DME, CNA services, and RN/NP visits as needed. We believe hospice is about building a relationship with the patient and family, not simply providing a service until the hospice benefit ends.”
