A Sierra Team Member is Available 24 Hours / 7 Days a Week
- Regular visits by registered nurses with specialized training and expertise in pain and symptom management.
- Visits by certified hospice aides to provide personal care attention.
- Direct access after-hours to the Sierra Hospice Triage RN.
- Enhanced communication with secure HIPPA texting platform for live communication and immediate attention to your needs.
- Psychosocial support for patients and families.
- Spiritual support for patients and families, including support groups.
- An in-depth intake/referral process, including fast response times for admissions.
- Trained volunteers who provide companionship, assistance and support.
- Unique rapid-response programs aimed at avoiding hospice readmissions.
- Direct, timely access to the hospice team for any concerns.
- Bereavement counseling and support for family members and caregivers throughout the patient’s illness and for a minimum of 13 months.
- Access to a grief and Recovery Specialist.
- Coordination of medical equipment and medications related to the terminal illness for comfort and symptom management.
- Physical, occupational, speech, music and other therapies as indicated in the plan of care.
